What Is the Medicare GUIDE Program?

The Medicare GUIDE Model is an 8-year pilot program for individuals enrolled in Traditional Medicare Parts A & B (not Medicare Advantage). It offers comprehensive, no-cost support for people living with dementia and their unpaid caregivers.

GUIDE provides:

  • • Funding for non-medical respite care so caregivers can take a break
  • • A dedicated dementia care navigator and interdisciplinary care team
  • • 24/7 caregiver helpline, education, and support resources

The goals of the GUIDE Program are to:

  • • Help individuals with dementia remain safely at home
  • • Delay or prevent nursing home placement
  • • Reduce avoidable emergency room and hospital visits

Greenstaff Homecare’s Role

Greenstaff Homecare is a participating provider in the Medicare GUIDE Program. We complete the initial eligibility assessment and guide families through the process of accessing benefits.

Our role is not to add work or complexity — it’s to support families and community partners by:

  • Assessing eligibility for GUIDE benefits
  • Connecting families to appropriate services and care resources
  • Providing ongoing guidance and navigation throughout the journey

Who May Qualify?

Families may be eligible for the GUIDE Program if:

The individual has Traditional Medicare Parts A & B, and

There is a diagnosis of dementia — or a need for an assessment to confirm eligibility

Our team offers a personalized and streamlined assessment to determine eligibility quickly and easily
